Outcomes Pharmaceutical Health Care “Outcomes” is a privately held limited liability company formed in January 1999 to become a leader in the emerging market for Medication Therapy Management Services (MTMS). The firm’s founders shared an insight that pharmacists were highly trained and underutilized experts in drug therapy. They also shared a conviction that, positioned with the appropriate systems, pharmacists could improve health care quality and reduce costs.

The Outcomes Encounter Program provides your health plan members with the knowledge and expertise of personal pharmacist. By presenting their Outcomes Card to any participating pharmacy, members may receive any of the following services to help them get the most value from their medications:
Outcomes Disease Management and Health Screening Programs
Outcomes Disease Management Programs provide intensive drug therapy management and monitoring services to health plan members with complex or rigorous medication needs. These programs provide for improved therapeutic outcomes and the tracking of significant disease markers by pairing specific enrollees with pharmacists specially trained in Disease Management.
Outcomes Personal Pharmacist Program™
The Outcomes Personal Pharmacist Program is your passport to the knowledge and expertise of a personal pharmacist. 58% of all adults have a personal pharmacist to help them with their medication needs. By presenting your Outcomes Card to any participating pharmacy, you may receive any of the following services to help you get the most value from your medication.
Outcomes Partners+Plus Program™
Partners+Plus is a new program that combines the data mining features of your PBM with the Outcomes professional services menu to create a powerful health care management tool. Through Partners+Plus, your PBM identifies members of a client's health plan consuming 7 or more prescription medications. A certificate is mailed to the home of each of these members, offering a Comprehensive Medication Review by a specially trained Outcomes Personal Pharmacist™. During the Comprehensive Medication Review, the pharmacist will assess the member's drug profile for potential conflicts, therapeutic duplications, and formulary/generic savings opportunities. They will then consult with the member and/or their physicians, as appropriate.
