Over the years, standards of practice have been implemented in hospital pharmacies - like unit-dose packaging and bar coding - to help reduce medication errors and increase patient safety. Pharmacies have typically looked to USP, ISMP and ASHP for suggestions on best practices and standards. As the hospital pharmacy environment continues to evolve, regulatory agencies are standardizing and formalizing some of these pharmacy practices.
